What insurance do you accept?
We accept the following insurances listed below:

• BCBS
• Health Advantage
• Ambetter
• Qual Choice

What is your fee schedule?
Appointments in the Office
Fees are $225 for the initial intake appointment and then $165 for each appointment thereafter. Checks, cash, and credit cards are accepted. Cancellations must be made 24 hours in advance of your scheduled appointment, otherwise the $165 cancellation fee will apply. 

Court Appearances
The Policy for Create Healing Counseling Services PLLC mandates that any court appearances from our therapists require a deposit of $2,000 from the client for each day the therapist is court ordered to appear. The deposit must be paid before the court date. Any time required by the therapist to travel to and from court, preparation for court, meetings, and phone calls related to court will be billed towards the client at an additional  rate of $250 per hour.

Requests for Records
Requests for records will be subject to a $100 administrative fee.

Out of Network Insurance Assistance
If you wish to file out of network with your insurance, we can provide documentation for you to do that.

As of January 1, 2022, The No Surprise Act mandates that we inform you of your rights if you wish to see a provider who is out of network with your insurance. Please see the linked form with details about your rights under this act.
