What is your fee schedule?
Appointments in the Office
Fees are $200 for the initial intake appointment and then $150 for each appointment thereafter. Checks, cash, and credit cards are accepted.
The Policy for Create Healing Counseling Services PLLC mandates that any court appearances from our therapists require a deposit of $900 from the client. The deposit must be paid before the court date. Any time required by the therapist at court after 4 hours will be billed towards the client at a rate of 150 per hour.
Requests for Records
Requests for records will be subject to a $100 administrative fee.
Out of Network Insurance Assistance
If you wish to file out of network with your insurance, we can provide documentation for you to do that.
As of January 1, 2022, The No Surprise Act mandates that we inform you of your rights if you wish to see a provider who is out of network with your insurance. Please see the linked form with details about your rights under this act.
